In response to the rapidly changing COVID-19 landscape, Champlain Community Support Network put together a dedicated COVID-19 Response Site where vulnerable and isolated seniors and adults with disabilities could request community support services that are available in our region.
The bilingual site focused on these key services:

Pharmacy delivery and help with food

Transportation to medical appointments

Telephone safety checks

Meals on Wheels

Dementia support

Staying connected while at home
Services were requested by residents as well as service providers. Individuals completed a simple online request form indicating the needed services and their location.
During the COVID-19 vaccination rollout, the site also offered a portal to book transportation to vaccine appointments in Ottawa and Renfrew County.
This site offered support to community members and our health and community partners to quickly identify and refer to the services they required during COVID-19 for vulnerable seniors and adults living with a physical disability.
The site also allowed for volunteers to sign up for specific roles during this heightened time. Their applications were routed to their local community agency after a high level screening, and they received training and onboarding to help our clients.
